At BenefitsLife we prefer an Experience-Rated Extended Health & Benefits Plan. That means your Monthly Premium will be based on how your own Group will use your Benefits and how Much. Most Plans allow you to spend between 64% – 83% of your Premiums on Extended Health & Dental Expenses.

So we need to know how many people, at what ages, sex, and coverage status (Single, Couple, or Family) will be covered by the Plan.

If you select Disability Coverage, we’ll need to know the incomes of each person insured. The Premium for Disability is based on income.
